To apply for water/sewer service please complete the form below. You will need to attach a copy or photo of 1) your driver’s license and 2) your lease or purchase agreement to the form below. Please make sure these are available on the device you are using.

Please do not submit application prior to two weeks before service start date.

At least thirty (30) minutes after the form has been submitted, please contact our office at 770-781-2160 to speak with a customer service representative, schedule your connection date, and pay your deposit and any other fees with a Visa or MasterCard.

Connections are made Monday through Friday only.

Red Flag Rules for Identity Theft Prevention Program

The Forsyth County Board of Commissioners, to ensure compliance with and support of the Federal Trade Commission Regulation 16 CFR, adopted County Ordinance 112 on April 16, 2009.  The "Red Flag" rules are an amendment to the Fair and Accurate Credit Transaction Act (FACTA) of 2003.  "Red Flag" references are cues to a pattern, practice, or specific activity that indicate the possible existence of identity theft.  A complete copy of County Ordinance 112 can be viewed by clicking on the link below.

Forsyth County's responsibility is to detect, prevent, and mitigate identity theft by identifying and detecting identity red flags and by responding to such red flags in a manner that will prevent identity theft. The processes of opening a new account, making changes to an existing account, and making payments on such accounts have been identified as potential processes in which identity theft could occur.  We ask for your patience as our staff members obtain, review, and verify certain identifying information in order to detect, prevent, and mitigate identity theft.

Identifying information you may be asked to provide can include name, residential or commercial address, principle place of business for an entity, date of birth, driver's license number, and other valid government issued identification, proof of residency, marriage license, divorce decree, and / or death certificate.
