Birth, Death, and Marriage Certificates

Requests for birth certificates, death certificates, and marriage certificates can be made in writing or in person.  Please contact our office at (770) 781-2140 or probatevital@forsythco.com  to request a form be sent to you. 

Birth Certificates

Complete the Application for Birth Certificate Copy which can be found under the forms section of the Probate Court website.  You may mail the request with a self-addressed, stamped envelope and the birth certificate will be mailed back out to you on the same day the request is received.  You may also come in person with the request and receive the birth certificate in person at the time of the request.  

You will need to provide identification with the birth certificate request.  Acceptable forms of identification include:  driver's license, passport, Georgia Weapons Carry License, military identification, employer or school identification card, or state issued identification card. Requirements for various relationship requests are listed below.

The fee is $25.00 for the first copy and $5.00 for each additional copy purchased at the same time for the same person.  Payments made be made with cash, check, or credit card.

Requests for own birth certificate or that of your child (as long as you are listed as a parent on the birth certificate)- provide your own identification.

Requests for birth certificate of parent or sibling- provide your identification and a copy of your birth certificate to prove the relationship.

Requests for birth certificate of grandchild- provide your identification and copy of your child's birth certificate (listing you as parent).  Your child must be listed as a parent on the birth certificate you are requesting.

Requests for birth certificate of spouse- provide your identification, a copy of spouse's identification, copy of marriage certificate, and a signed notarized letter from your spouse giving you permission to obtain the birth certificate.

Death Certificates

Complete the Application for Death Certificate Copy which can be found under the forms section of the Probate Court website.  You may mail the request with a self-addressed, stamped envelope and the death certificate will be mailed back out to you on the same day the request is received.  You may also come in person with the request and receive the death certificate in person at the time of the request.

If you need a certified copy of a death certificate, you must be an immediate family member (parent, adult sibling, adult child, spouse) and show proof.  If you are not an immediate family member, you may only receive a copy that does not include the social security number, cause of death, or any certification.

To obtain a death certificate for your spouse or child-  provide your identification.

To obtain a death certificate for your parent or sibling- provide your identification and a copy of your birth certificate to prove the relationship.

Marriage Certificates

Certified copies of your marriage record can be obtained any time after the original license has been filed with the Court.  Typically a certified copy of the marriage certificate is needed to change your name with social security, driver's services, passport, immigration applications, and foreign adoptions.  You may also need to provide a certified copy of your marriage license with the marriage certificate for certain agencies.

Complete the Marriage Certificate Application Request Form which can be found under the forms section of the Probate Court website.  You may mail the request with a self-addressed, stamped envelope and the marriage certificate will be mailed back out to you on the same day the request is received.  You may also come in person with the request and receive the marriage certificate in person at the time of the request.  

The fee is $10.00 for each certified copy.  Payment can be made by cash, check, or credit card.
