Subdivision - Development - Road Name

How do I submit a subdivision/development and/or road name request?

Applicant must read the Forsyth County Department of Engineering Subdivision and Road Name Policy and Procedure.

Then download the Request for Road Name Approval form and complete the form online. You may move to each field by using the F11 key. When the form is complete save it to a word file. You may then attach it to an email and forward it to Lisa Rodgers at the Forsyth County Engineering Department larodgers@forsythco.com or Lisa Phillips at lmphillips@forsythco.com. The form may also be faxed to (770)781-2104.
